Here in Cloudcroft, an internet provider ran fiber to EVERY house in the village to the wall. Didn't matter who, whether or not they were a subscriber. And if you wanted it turned on, they'd come out and run a Cat5 from their box into the interior and you were good to go.
We've been using their service for several years, quite reliable. Somewhere down the line they upgraded our service from 25/25 to 50/50 with no change in fee.
