I don't know why it came to mind, but I remember an old Clint Eastwood movie, Gauntlet (IIRC) that was filmed in Phoenix at Symphony Hall, which stood in for Police HQ and City Hall. They crashed a bus up the steps and did a lot of damage to said steps. ANYWAY, a police friend of mine was working security on the set. Lots of extras in police uniforms, and lots of actual police working security on the set. Eastwood was a cop who got in trouble with his buddies, has to drive a bus up the steps and gets shot by a cop. The director wasn't happy with the way the actor shot Clint, so he pulls a gun out of the holster of one of the extras and says 'No, shoot him like this!' Except the extra wasn't an extra, it was a real cop with a loaded weapon. The shout of 'GUN!' went up and the real cops dog-piled the director. No one got hurt except the director's dignity.
