I'm sure a vast majority of places don't bother with fragment checking. I haven't tested yet, but I expect that if you OCR a Word->PDF conversion that you're probably 99% gold, but scanning something like a book is another matter entirely. And most professional orgs, or at least unis, have Acrobat Pro, so not much of an excuse not to do fragment checking except not wanting to spend time or being ignorant on the issue.
